To download the application form click the above "Download the form" link. You will be getting a .DOC (MS-Word 2000) file. Send the printed copy of the form (filled up properly) along with the followings to the University

	1.	DD of Rs. 700/- in favour of West Bengal University of Technology payable at Kolkata. For SC/ST candidates the amount of DD is Rs. 350/-.
	2.	Attested photocopy of Madhyamik Admit Card
	3.	Two copies of stamp size photograph duly signed on reverse side.

For any future correspondence / obtaining admit card you have to refer the following information :

a. Name of the Candidate
b. DD No.

It is recommended that you keep a photo copy of the filled up application form for your own reference. Please note that for obtaining the admit card you have to refer the above information (a,b) exactly the same way you have mentioned in the form.

The last date of submission of form along with documents is 21.07.2008 . Personal submission is also possible between 12:00 - 4:00 p.m. on working days.
